Self-change request forms allow employees to submit requests about themselves directly in the system.
This is not limited to personal data updates (such as phone number or address). You can use this type of form for any employee-related request, depending on your company’s needs. Examples include:
Personal data changes
Bank details updates
Document requests
Internal transfer requests
Termination (resignation) request
In this example, we will set up a Resignation request form. This allows employees to submit a resignation request directly from their profile, while HR receives the information in a structured and organized way — and can then take the necessary next steps, such as:
Reviewing and approving the request
Scheduling an exit interview
Starting the offboarding process
Preparing final payroll and documentation
Discussing a possible counter-offer (if applicable)
Step 1. Create the fields employees will fill in
First, create the questions that will appear in the form.
Go to Settings → People data fields.
Create a new group (for example, “Termination request”).
This group works like a folder for all resignation-related fields.Add the following fields (you can adjust them to your needs):
Suggested fields:
Planned last working day
(From which date do you plan to end your employment?)Reason for resignation
(General reason)Additional comments (optional)
Open to counter-offer?
(Yes / No)If yes, what could change your decision? (optional)
(For example: salary review, role change, remote work, etc.)
Once these fields are created, they can be added to the form.
Step 2. Create the self-service request form
Go to Settings → Core → Forms → Self-service request.
Click to create a new form.
Name the form (for example, “Termination request”).
Create a section and add the employee fields you created earlier.
5. Add Approvers (for example, HR Manager or a specific person).
✅ Your form is now created. 🎉
Step 3. Decide who can see this information
Now choose which roles in the company should see these resignation details.
Go to Settings → Roles and Permissions.
(Administrators automatically see all fields.)For each role, you can choose:
No access – hidden
View only – can see but not edit
Edit – can update
Example: Manager role
If managers should see resignation requests of their team members:
Open the Manager role
Go to People
Find the Termination request field group
Change from No access to View only (recommended)
Save changes.
You can repeat this for other roles (for example, Team Lead or HR Manager).
Step 4. Allow employees to submit the form
Finally, give employees access to use the form.
Go to Roles and Permissions → People about themselves role → People → Forms.
Find Self-service request and check the box.
Save changes.
Result
Employees can now submit a resignation request directly from their profile using the Action button. HR receives structured information, the process becomes transparent, and manual emails are reduced.
