The Termination forms feature is now available in Settings → Forms under the new form type Termination form. It helps HR teams and managers handle employee offboarding quickly and accurately.
You can customize termination forms just like other forms - add employee fields, request documents, and include approvals as needed.
For managers with termination permissions, this feature makes the process more efficient: they can terminate employees or direct reports directly from their profiles using the form, while the system automatically updates employee records, profiles, and lifecycle status, and triggers offboarding workflows.
Access termination forms
Go to Settings → Forms.
You’ll see a new form type called Termination Form.
By default, there’s a pre-configured form that already includes all the required system fields - such as Last work day and termination details. These fields cannot be removed.
Optional fields, like Last day in office, can be added or removed at any time depending on your company’s needs (for example, they may not be needed for remote employees).
Create or edit a termination form
You can easily adjust the default termination form to collect all the information you need, or create a new one from scratch.
1. Name and describe the form. Give your termination form a clear name and description to make it easy to identify, for example Immediate termination.
2. Set up workflow automation. Choose the workflow you want to trigger automatically once the termination form is approved or submitted — including cases where forms are auto-approved (if approvals are not enabled in the form settings).
3. Add extra fields. Include employee custom fields or fields from the Personal tab to capture any additional details you need.
4. Define required fields. Decide which fields must be filled out and which are optional.
5. Add an employee document block so users can upload files — such as resignation letters, agreements, or other offboarding documents — directly in the form.
Set up the approval process
If your termination form requires approval, enable the Approval section when editing the form. Then choose who should approve the request. You can pick from three types of approvers:
Personal: Includes employee-related options such as Manager, Manager’s manager, 3rd level manager, or Team lead(s).
Department: Includes the Department manager and any custom fields from the department setup.
Specific person: Lets you select a specific employee by name from a dropdown list.
The approval chain will automatically apply to the employee the form is being submitted for.
Additional settings:
Skip unassigned enabled: If no specific approver is assigned for a step, that step will be automatically skipped.
Substitution enabled: If an approver is on leave, the approval request will be forwarded to their assigned substitute.
How to terminate an employee using termination forms
Only users with termination permissions (such as managers or HR administrators) can initiate or approve terminations.
Go to the employee’s profile, click Actions, and select Terminate Employee.
Select either the default termination form or a custom form you have created.
Fill in all required fields and add any optional information if needed.
If the employee has direct reports, mark the checkbox to reassign them to a new manager.
Check the box if you want to stop any ongoing workflows the employee is involved in.
Review all entered data and the changes that will be applied. Once submitted and approved, the employee’s profile and lifecycle table will be automatically updated.
✅ Termination requests are sensitive, so they do not appear in the Requests → Company tab. All submitted termination forms can be found under Requests → My tab.
Approve, Decline, or Send the Form for Revision
Managers can submit termination forms. If the form requires approval (as defined in the form setup), it will be automatically sent to the designated approver.
The approver - such as HR or another role defined in the form settings - will receive a To-do item and a system notification. From there, they can:
Approve the form,
Decline it, or
Send it back for revision.
For immediate terminations, the configured offboarding workflow will start right after the form is submitted or approved, ensuring that the process begins without delay rather than waiting for a scheduled start date.
Termination vs custom forms
Termination forms directly affect employee records, while custom forms are only used to collect information.
Custom forms store data but don’t update employee profiles or start any workflows.
Termination forms, however, automatically end an employee’s employment, trigger offboarding workflows, and update all related records.
